Understanding PROs (Patient-Reported Outcomes) in Oncology
In modern medicine, particularly in oncology, traditional clinical assessment (performed by the physician via scores like ECOG or Karnofsky) is no longer sufficient. Patient-Reported Outcomes (PROs) represent a fundamental paradigm shift towards Patient-Centered Care. A PRO is defined as any report of the status of a patient's health condition that comes directly from the patient, without interpretation of the patient's response by a clinician or anyone else.
Why Give the Patient a "Voice"?
Decades of clinical research have highlighted a recurring phenomenon: symptom underestimation. Clinicians tend to grade chemotherapy toxicity (nausea, fatigue, pain, anxiety) less severely than what patients actually experience. Integrating PROs bridges this gap. The main objectives are:
- Improve Quality of Life (QoL): By identifying unspoken symptoms early, the medical team can adjust supportive care.
- Increase Survival: Major studies (such as those by Basch et al. presented at ASCO) have proven that regular electronic monitoring of PROs significantly prolongs Overall Survival in patients with metastatic cancer, simply because complications are detected and treated earlier.
- Optimize Clinical Trials: The FDA and EMA increasingly require PRO data to validate the approval of new anticancer drugs.
The Reference Instrument: EORTC QLQ-C30
This interactive tool is a simplified version inspired by the official EORTC QLQ-C30 (European Organisation for Research and Treatment of Cancer) questionnaire. The full questionnaire includes 30 questions evaluating physical, role, emotional, cognitive, and social functioning, as well as global health status.
The Future: Wearable Devices and Connected Health
The logical evolution of PROs is their association with wearable devices. While PROs capture the patient's subjective experience (how they feel), smartwatches and activity trackers measure objective data in real-time (heart rate, sleep quality, step count). Combining these two data streams (subjective + objective) offers the oncologist a perfect, continuous map of the patient's tolerance to treatment.
