Initiating anticoagulant therapy (VKA or DOAC) in an Atrial Fibrillation (AF) patient is a crucial decision to prevent ischemic strokes, but it inevitably carries a bleeding risk. The HAS-BLED score is specifically designed to objectively assess this 1-year major bleeding risk.
What is the HAS-BLED Score?
The HAS-BLED score is a globally validated clinical risk assessment tool. In this context, "major" bleeding is defined according to ISTH (International Society on Thrombosis and Haemostasis) criteria: fatal bleeding, symptomatic bleeding in a critical area or organ (e.g., intracranial), bleeding causing a fall in hemoglobin level of ≥ 2 g/dL, or leading to transfusion of two or more units of whole blood or red cells.
Major Warning (ESC Guidelines)
There is a common clinical pitfall regarding the use of bleeding risk scores:
- A high HAS-BLED score (≥ 3) is NOT a contraindication to oral anticoagulation (OAC). It should never be used as an excuse to withhold or stop treatment in a patient at high risk of stroke (e.g., high CHA2DS2-VASc score). The net clinical benefit of OAC remains largely positive, as preventing a disabling stroke outweighs the risk of bleeding.
- The primary utility of the HAS-BLED score is to identify patients needing closer monitoring and to encourage physicians to aggressively correct modifiable bleeding risk factors. This includes: lowering blood pressure (target systolic < 130 mmHg), stopping NSAIDs or antiplatelets if not formally indicated, reducing alcohol consumption, treating anemia, and improving Time in Therapeutic Range (TTR) for patients on VKAs (or considering a switch to a DOAC, which is often safer).
