Interpretation & Prognosis (UNOS)

The MELD (Model for End-Stage Liver Disease) score is the internationally validated system for prioritizing patients awaiting liver transplantation.

What is the MELD-Na score?

Originally developed in 2001, the classic MELD score uses bilirubin, creatinine, and INR. In 2016, the UNOS system officially integrated Sodium (Na) into the equation, as hyponatremia is an independent predictor of mortality in cirrhosis. This calculator uses the standard MELD-Na equation.

Official Calculation Rules (UNOS)

The calculator automatically applies the following limits:

  • Values < 1.0 (for INR, bilirubin, and creatinine) are rounded up to 1.0.
  • Creatinine is capped at 4.0 mg/dL. If the patient is on dialysis, creatinine is automatically set to 4.0 mg/dL.
  • Sodium is bounded between 125 and 137 mEq/L for the corrective calculation.
